If you have 2 cards and not sure which one you are going to use then I suggest print 2 copies of payment page and fill in for different cards. Thanks, visa was granted in Jan 2012 (valid till 22/11) and she entered the country in March 2012.

As you entered UK within 3 months, you can count your time period from visa stamp date.

You are not required to give any other doc evidence but give ref to the guidance to be safe.
