I chose Other Main Applicant since I was on a work permit but I guess you would choose Tier 1.

I just had my employer note absences for work and state that during annual leaves, I was still paying NI and getting salary payments. Self-certify your personal absences on the form. As long as they're not greater than the limit, it should be okay.

Just to update:

I've received an email from PEO Refunds asking for my application details and date, venue, and time of appointment that I want. So, hopefully this will get us an appointment.

I've just gone through the same thing. UKBA has taken £3750 and I have no appointment for my ILR appointment. We went through the booking with no issue, then went to Worldpay to pay for premium service, but when the Worldpay site sent us back to UKBA the latter site had logged us out for inactivity....

But beware of timeouts of online activity while you're redirected to World Pay. We are currently out £3700 and we have no appointment. And now my CC is maxed-out. Now, I need to wait for anyone to answer my emails! So, I would say it's very difficult and nightmarish, in my experience

I am in a similar position but not as bad- I have letters from june 2011 then none until 2012, I know the June ones are a little out of the 24 months. It's nice to hear, Amber, that set(o) is not as strict with regard to cohabitation documents.

The other thing is there is no UK exit stamp a lot of times, because I hate leaving through London, so I'm usually travelling via Amsterdam to US. So, there's that opportunity for a stamp gone! Frustrating!

Sudarsanam, That seems like enough. I don't think you need P60s but the form typically (depending on application type) asks for proof that you were working, so bringing any P60s you have would be ideal for showing that you worked in the last 5 years.
