Birth Certificate issue

Hi All,

I had been issued a Birth certificate from the Municipal Authority without my Name.....It just contains the details of my parents name & my Date of Birth,Sex.

Can i use this certificate for my HSMP application or should i just go with my Driving License ?

Thanks Prem, But the ITR which i will be sending is a consolidated ITR of both the Companies....showing the details of both company X (9 months )& Y (3 months )....& since Company X is closed now So i dont think i need to fear about that as i am not showing that 9 months of Company X in my total 12 ...

The above case workers document says that stamped Saral is acceptable & Form 16 is NOT.
