Thank you very much for your prompt advise. Really do appreciate it. I initially thought I should use the SET (M) in such case, but now reading your comment and the form again, SET (O) appears to be correct form to use. Thanks,

Thank vinny for the quick response.

But if I get a ILR and get a biometric before her visa expires and she passes the LIUK test, then which form should she use to apply for her ILR? Can this be done outside UK?

Not necessarily. They could send the same template regardless of outcome? Just because a cow has four legs and a table has four legs doesn't mean a cow is a table. I agree. Thanks for that. I thought so, it was my impatience! I just hoped it clearer as I thought, how can they dispatch visa, unless they approved and given visa upon our application! I guess I just need to wait and see!

I provided same evidence as my usual wage.

1. bank statement, showing of the receipt of the bonus
2. Payslip

The payslip's payment date and amount should match of the receipt date and the amount in the bankstatement.
