MN1 Form Referees And Identity

Hi,

I am applying for my child 18 months old. Two referee required. One of them should be professional such as teacher, health visitor, social worker or minister of religion.
I can request health visitor only as she might know my son in professional capacity. But she has nicely denied to sign.
What could I do for professional referee?
