MN1 Section 1(3) Queries

Hi All,

Please if someone can help me these queries rearding MN1 form

1.3) The child was born here and parent have received ILR on 07/01/2015 , so what date we need to write in this section or should we leave it blank as child has never applied for ILR but has got Visa till Apr 2015

2.1) If the child is resident in UK , please give date aand place of first arrival?
What date to write as child was born in UK

Section 4 Good Character Requirement
The child's age is 4 year do we need to fill in this section or leave it blank completely.

Section 5 Refrees and Identity
We know a doctor very well but she is not our childs GP.
Can she sign as a professional referee?

Section 7 Declaration
7.8 ) As the parents got ILR on 07/01/2015 so they are not eligible for naturalization but the child is , do we need to tick 7.8?

Is it better to use NCS ( Standard or Express) or its better to send the application by our own.
