17 months Old Son Mr or Master

Hi,
I am filling MN1 form .What I write the title of my 17 months Old son.
Either tick the Mr box
or write Master in Other title box.

And what I write in the box child,s Mother's Full name & Maiden Name
If suppose
After Marriage her current name is Mrs Shahida Waqas Ahmed
and before Marriage her name was Miss Shahida Aslam Cheema
