Name wrong in the family visitor visa

Name wrong in the family visitor visa

Just realised that the family visitor visa I applied online from had a mistake. In the declaration form, name of the form completed by is wrongly appearing as my email id instead of my name as in the passport.

This is visa is very important and I can not risk the delay or rejection. Please could anyone suggest the best option.
