Naturalisation Query - Name Formatting and Documents

I would appreciate it if someone could help me with the below queries.
I am going to apply for Irish citizenship based on 5-year residency.

1. Name formatting:
My First Name is AA BB and my middle name is CC DD.
My current passport has my First name as AA BB CC DD and my last name is blank.
The name in IRP was captured as FN: AA BB and LN: CC DD

How do I fill my name in the naturalization form? Will it be as per my current passport or IRP as the last name in the form is a mandatory field?

2. I am going to submit the below documents with my application - hope these are enough
Identity Proof:
Bio page from my current passport - Certified by a Solicitor
Certification Form

Residency Proof:
Type A
Last 4 years Employment Detail Summary - No need for certification by a solicitor
3 months' bank statement for the current year

Type B:
4 years Electricity Bill
1 year Rent Agreement

Am i missing anything?

I don’t think there’s any standardised format. In the Visa application they instruct people to put a dot “.” when no surname or first name is available. You can also use the format in your IRP card as that is the version of your name in the system.

You will need to write a quick cover letter to explain the discrepancy between your passport and your IRP card anyway.

And you are missing the certified copy of birth certificate.

I will include a cover letter and my birth certificate.
One last thing, I am assuming that we need to provide only 1 month bill per year and not necessarily multiple, correct?

They did mention that they want 3 consecutive months of bank statements, but they didn't say anything regarding utility bills.

I submitted 3 consecutive months of utility bills (if you are billed monthly, get 3 bills; if you are billed every 2 months, then get 2 bills) when I applied for my application and my missus's application.
