Need information FLR-M form fillup

Hello Gurus,

I need your guidance to fill up couple of information for FLR - M form. i got appointment for 25 July 2018. please i need your help. i am company director and my wife is employee in same company. which category do i fall for this situation.

I think Category F ? is it right and do i need to fillup all this information ?
