New application form

New application form

I am preparing my application. I noticed that the HSMP have used new application forms since Jun.28,2004. But I have examined the new form and old on, there is not any change in content. Just the size of the file changed from 486K to 229K. Anyone can tell me do I must use new forms? What is the difference between them?

New one is valid, the old one is not, there could be other differences but in any case its best to use the new form.

Sure it would be best to print the new form. However if you download the new pdf and compare create date with the "old" one - it is just the same. And I don't know how this came for you but size is the same. 400+k is just the guide.
