New passport for my Kid- Do I need change in Apearance

Hi All,

I have to apply changed name passport for my kid, do I need to submit “Change is Appearance” notarized affidavit, which I got to know just now, tomorrow I was planning to apply in Birmingham HCI. Is this the mandatory document ? Current passport was acquired when he was 4 months old, now he is 4 years old. Is there any easy or quick way to get it.. just in case if any one has any idea ?
