new/renew indian passport for 6yr old child

hi,
i need to renew/apply for new indian passport for my daughter which expired 11th november 2010,would that include getting the change of appearance(she was 8months when it was made in india)form signed by notary public or will it be ok just to send it straight by post.Any idea how much they charge for notary signature.
