NHS flr(m) overstaying

Another quick question is it illegal to register with NHS GP having overstayed a visitor visa and awaiting a reply for another application for a diffrernt visa?

I am not sure that illegality comes into it, but that is certainly no entitlement to use NHS GP services.

Breaching NHS regulations, yes.

Breaching the Immigration Rules? No, but that is only because there is nothing at all in the Immigration Rules about the NHS.

Generally, when you register with GP, they ask for passport details+visa page and current address. If your passport shows overstayers, they should not register you?
