NHS Surcharge on the form

Hi Folks,

I am extending my Tier 2 General Visa using the priority service. However I am unable to figure out where do I put the IHS reference number after I pay the surcharge in the application form. Will it be in the section H of the form ? or do I attach a letter along with my application?
