My son AAA has no surname in India passport but as it is Mandatory for UK passport to have surname so we included part of his name in the UK passport as surname.
His name in India passport is BBB AAA and in UK passport we added BBB as Surname and AAA as First name so his name has now become AAA BBB in UK passport.
We have got a response back from HCI to submit an affidavit for name change for him from BBB AAA to AAA BBB.
Can someone advise if we have any template we can use and also if the deed can be done anywhere cheaper as I received a quote of £200 In Kent and they also don't have a template that can be acceptable by HCI. They have asked me to get the template and content populated
