old visa refusal

I have following questions regarding previous visa refusal question in the application

• Is there a time limit which resets the clock for previous visa refusals (I read somewhere any refusal over 10 years shouldn't be mentioned)? If yes can someone please provide a reference?

• If a visa was refused and was given right of appeal however the post decision team reviewed the case and reverted the decision. Does it count as a refusal ?

2. This is exactly what happened to my friend, his visa was refused and was eventually overturned by the court. He declared this as refused and overturned in his application form.
