Applying Indian Passport Re-issue (Renewal) for minor from Uk London

Hello All,

i'm UK citizen(via work permit), my son & wife are holding indian passport as they are in Dependent visa running 5th year.
my son born in india while i was holding ILR
My son passport expired recently(Nov 20, 4 months before) & got appointment to reissue his indian passport at London office.

my question is when i submit his application, i have to submit Police veridiction form for 3 of us

Could someone please advise me the following my doubts?

for them in Police Verification Form

1. Should i mention Permanent Address to "Indian Address" & Present address to "Current UK address"?
2. Referee names should i give Indian referees to both or one Indian & one uk referee?

for me (UK Citizen)

2. Declaration form
3. Annexure E
4. Police Verification Form (3 forms) one for me, one for my wife & last one foe my son as mentioned above
5. Appointment Confirmation Printout
6. Current Indian Passport
7. Residence permit
8. Annexure D
9. Marriage Certificate and Spouse’s passport:
10. UK Address Proof (Utility bills)
