Re-issue of Indian passport Affidavit

My wife will be applying for a re-issue of her Indian passport and will be taking my surname. She arrived to the UK on a spouse visa in December.
We have started filling in the online application. My question is regarding the affidavit. Is the “change on name” affidavit required and if so, can it be notarised at the VFS centre?
