Passport Surrender

Could someone who applied for surrender certificate help me with the answers?
Unfortunately, the request is urgent as I am filled up on a cancelled appointment today.

1. On the application form, under permanent address - Should this be UK address or the Indian address on the Surrendering Indian passport?
2. Should I buy Special delivery envelope or is this something VFS will provide for a charge? Alternatively, Can I go in person to collect the required document from VFS?

Applying at Edinburgh VFS today.

I haven't got the response but thought I will put down the answers so that someone will be benefited.

1. The address should be the current UK address that you live and the address that your surrender certificate should reach you.

2. They are charging around 15£ for the special delivery envelope / DX to post the surrender certificate back to you. I found it expensive and I made a mistake not to take a special delivery envelope over the counter from Post office. Anyone going for passport surrender please do buy an A4 size special delivery envelope from Post office without fail to save some money.
