PBS dependant visa for wife and child

Hi,

1. Please help as while filling my Wife's application form I had 5 options, and I chose UAN number
UAN,
GWF
VAF
Home Office reference number
No reference number

However , for my child application form only 4 options (except UAN Number) and I don't have GWF, VAF, Home office number. What should I do. Please help.

2. Letter of consent: How can I provide consent while my child is in India. I have given both are legal guardians responsible for my child. Please help.
