PBS Dependent for Tier 1 General

Hi,
I Hold Tier 1 General. My five years on Tier 1 General will complete 04/06/2016 but before that I will complete 10 years of my legal stay in UK on 09/09/2015. I am planing to apply ILR based on lawfully completing 10 years on uk in August 2015. I've also just got married in Pakistan on 08/10/2014. I want to apply for my wife dependent visa and bring her to UK. I have following few questions.

1. Is "POINTS BASED SYSTEM FORM (VAF10 DECEMBER 2012) DEPENDANT" the right form to fill?
2. What is the fee when Applying a dependent visa from pakistan? I think it is 14000 Rs.
3. What will be the length of my wife's dependt visa?
4. If my wife gets her dependent visa now and I will switch my status from Tier 1 General to ILR after 10 months does my wife have to reapply for her dependent again.

I will appreciate your reply and if you can kindly suggest any reading material as well. So far I have gone through POINTS BASED SYSTEM FORM VAF10 form, VAF10 Guidance, PBS (Dependant) POLICY GUIDANCE and documents-t1-gen-dep.
