Please help: Lost Passport/Visa

Hi,
I have a Tier 1 PSW visa issued last year after I finished my University Degree(Masters) in the UK graduating Dec09 and it is due to expire in March 2012.

I travelled home to the USA last week and I have lost my passport/it was stolen.

I have reported it, but yet to receive police report.

I have since got my new passport from the Passport office in San Francisco.

The visa was in the old one that i do not have. I have read that I needed to apply online through Worldbridge/visas4uk and have done the online application and paid the $562. I have assumed this to be the right method as a simple vingnette transfer is not possible as teh old passport is lost. I also have my biometrics appointment on Tuesday.

What other forms etc do I actually need? VAF9, and supporting documents from now; OR the original ones i sent last year, or both? What happens as my date of qualification is clearly over 1 year ago??

So confused and freaking out!
