Pls provide checklist for psw dependent visa !

me and my wife are in uk and we both are holding psw visas. but my visa is running out very soon and my wife's visa has about 8 months left. So now i want to apply the dependent visa and i am wondering if you can provide the checklist for psw dependent visa application. Also i want to know that, at the time to apply for dependent visa, how many months on her visa (main holder) should be remaining? thanks guys...!
