Passport expires this summer, but visa lasts until 2016

My Tier 2 visa lasts until spring 2016. The stamp is in my passport, and my passport expires this summer so I need to apply for a new passport soon.

What is the procedure for getting an updated stamp from the home office to put in a new passport?

How long does it take to get the visa put in a new passport?

you dont have to transfer the visa to new passport. Just keep the 2 together.
