your wife's birth certificate should show both the mother and daughters names.

Yes.
marriage certificate copy plus spouses passport copy if Indian that should state parents names. else birth certificate copy of spouse which can show her mothers name and that links you to your mother in law.

Hi, thanks for the reply. I can provide my spouse passport copy but the problem is name in my spouse passport

Mother's name in my spouse passport : Venkata Mahalakshimi
Name in mother-in-law passport : Venkata Malakshuvamma

also, i noted there is no option in online form to provide other names you were known as. so what i need to do here?

I believe your spouse given her mothers name differently when applying for her passport.

If you skip her parents details page in passport, would you have other documentary evidence, like spouse's birth certificate.

Else submit but in addition to birth certificate have some notarised affidavit made stating both the name variations are in fact single person and get those details pages notarised etc.
