PBS Dept appt booked before April 6 - NHS Surcharge reqd?

Hi,

I have got my ILR approved last week.
For my wife I have booked a Solihull PEO appointment for 28th of April and as I booked the appointment before April 6th, I was required to pay only the old fee for her dependant visa.

1. Now regarding the NHS surcharge, does it need to be paid for her now eventhough the appointment is booked before the rule change?

2. In the new application form dated april 2015, I don't see the means of giving the details of the NHS surcharge anywhere(as far as I checked), if its needed where do we need to give this info?

Because in the Section M5 of this application form where it asks about the category of the main applicant, there is no option for ILR. If this the right application form what should I select in M5?
The options are
Tier 1 (General) - go to M6
Tier 1 (Entrepreneurs) - go to M7
Tier 1 (Graduate Entrepreneur) - go to M10
Tier 1 (Exceptional Talent) - go to Section N
Tier 1 (Investors) - go to Section N
Tier 1 (Post Study Work) - go to M7
Tier 2 (General) - go to M11
Tier 2 (Intra Company Transfers) - go to M11
Tier 2 (Minister of Religion) - go to M11
Tier 2 (Sportsperson) - go to M11
Tier 5 (Temporary Worker) - All categories - go to M11

this is what got me confused:

For visa applications made outside the UK, you’ll have to pay if:
you’ve applied and paid your visa fee on or after 6 April 2015

as i have not applied yet and only have booked.



Can someone please answer the point 3 for me.

She did not make a visa application outside the UK.

3. PBS dependant form appears to be correct.
