NHS Health Surcharge

To all gurus and forum members!

Do we have to pay 200 pounds a year NHS Health surcharge at the time of extension for tier 1 entreprenuer visa inside uk?

I have checked following link gov.uk website about this new implementation and to my understanding it seems that we have to pay this new implemented fees from 6th april 2015.

Do you agree with this that we have to pay as part of our extension application? But there is nothing mentioned about this on new application form?

yes 200 per year per person for the duration of visa to be applied.

@CR001....thanks for sharing new application form.

Do we have to pay dependent child as well who is under 1 year at the moment? On the governemnt website it says following;

You still need to use the service to get an immigration health surcharge (IHS) reference number but you won’t need to pay if:
"you’re a child under 18 who has been taken into care by a local authority"

Does it mean children under 18 exempt from it?

No it doesn't. "you’re a child under 18 who has been taken into care by a local authority". You're looking after your child, they haven't been taken and placed into local authority (Council) care. You'll have to pay the surcharge for them.

if I am not wrong, in some cases, its not just £200, it could be as much as £450. If you fill the application form and it will tell you the exact amount of surcharge to pay.

Yes, it is £200 per year per person, payable upfront at point of application for the duration of what the visa extension will be so for example if Tier 1 Ent ext for 3 years for 2 applicants it will be £200 X 3 = £600 X 2

Wish there was an option to opt out. I have health insurance from my home country which covers my healthcare costs here, so I don't even use the NHS.
Maybe I should cancel the insurance and start using NHS then.
