90 days concession and 28 days rule - Clarification

I am Tier 1 General and will be applying for ILR (SET0). I am bit confused with calculating continuous period and 28 days. I have read attached document but not 100% sure.

•First Visa stamp and clearance is 15 Oct 2010
•First time enter to UK on 15 Nov 2010
•Current Tier 1 expiry is 19 Oct 2015
•I have got PIO appointment for 19 Sep 2015 for my ILR application
•I was never out of UK for more than 180 days in each of the five year.

Is 19 Sep 2015 appointment date for ILR application is good and have I completed 5 Years by 19 Sep 2015 ?.

Please advise.

What date I shall put in Start Date.

Is it First Visa Stamp Date or
Current Tier 1 Expiry Date or
PIO appointment Date

your start date will be your date of entry.

As you entered within 90 days from date of visa issue, so you are eligible to apply by 15/10/15 minus 28 days.
