Urgent! T1 Gen,Main app on ILR now,Dept need extension??

Hi Experts!

Next week POE appointment!!!

I was on Tier 1 Gen and now on ILR. Time frame below;
Wife got Tier 1 Gen dept visa 31-Aug-2013 (outside UK) till 15-Nov-2015
She needs one more extension to qualify for ILR (5 years rule).

1) Will I use the PBS dept form (Yes/No) ?

We both working (Me self-employed) wife (Perm).

2) Have joint account to show £630 maintenance funds (Yes/No) ?

3) Do I need show my previous earnings ie From April 2014 to April 2015 or this year ? This includes (Accountant letter, Payslips, Dividend vouchers, bank statements, P60) (Yes/No)

4) Wife is also working, Can we show combine earnings (Yes/No) ?

we do have other documents ie Council bills/Utility bills/GP letters etc.

when are you going for an extension of your spouse ?
