NHS surcharge

My wife is on flr m and has paid 500 for NHS surcharge for the second period of flr. She found an article that says that the surcharge is a deposit and that if your NHS costs exceed 500 , you will be charged more. Can anyone confirm if this is true please?

I just wander if you could know how to get the refund for the IHS please?
I payed online amfd I have the email the reference everything .
The problem is that I need to do one day service(to book the appointment) so instead of asking for the reference number they direct me to the online payment. ;(
