Letter of Consent for Tier 2 Child visa needed or not?

Hi, I am going to apply for a Tier 2 (General), while my wife and daughter (around one year old) will apply for a Tier 2 Partner and Child visa, respectively. We will travel together to the UK. In this case, during the visa applications, we need to provide a letter of consent for my daughter to support her application? I have this query because I received the appointment confirmation email stating "if you are under 18, a letter of consent from a parent or guardian" for the documents to be brought for the appointment.

Only required if the other parent is not applying at the same time as the child.

For example, my wife's first passport (now she carries her third passport) was replaced around 9 years ago. She actually can not remember if it was lost or retained by the passport issuing authority. So I select "other" for the question is this document lost, stolen or in your possession.

Also, in my wife and daughter's forms, I am listed as the person traveling with them. There is a question about one visa held. I answer no. Because I also need to apply for Tier 2 general visa to travel with them, even if I am holding a short term visit visa.
