Regarding Indain passport application form

Hi,

My baby born in UK, we have applied Indian birth certificate and we are planning to take Indian passport, when i tried to find application form for Indian passport in online i am getting 2 types of forms, i got confused which form is correct one, if possible can you please come one give me correct link for the Indian passport application form
