Child Benefit Required Documents

Applicant is British Citizen and is applying in their name, the child is on Settlement Visa.

What documents are required to be sent with the application for child benefit?

1 - Childs Passport
2 - Childs Original Birth Certificate from country of birth
3 - Childs BRP showing 2.5 years leave - is photocopy acceptable for BRP only?
4 - Applicants Photocopy of British Passport?
5 - Childs proof of address?
