Tier 2 - PBS Dependant child visa consent letter

Dear experts,

Me and my wife are in UK on Tier 2 general and tier 2 dependant, but my son is in India and we are planning to bring him here next month. He was born in UK but went to India back with his grandparents when he was 3 months old, Either me or my wife will be travelling with him when he is coming back. I filled in the application form and in the final application checklist I got a list in which below is one of items in the list.

A letter of consent that confirms:

• the relationship between you and your parent(s)/legal guardian(s)

• consent from your parent(s)/legal guardian(s) to the application

• consent from your parent(s)/legal guardian(s) to the living arrangements in the United Kingdom and the address where you will be staying

• whether one or both of your parent(s)/legal guardian(s) have responsibility for you
  Both parents/legal guardians must sign the letter, except where only one parent/legal guardian has sole responsibility for you

Could some one please help me with an example letter, I tried searching and I could not find any example letter or any letter template. Any help will be much appreciated.

You can use something like this. Make sure you will cover all the points you have mention above.

I know this question has been answered but please I still need help. In our case the father is already in the UK and I am in Nigeria. Since both parents are required to sign the document I would appreciate if someone in the house can help me out.

Can you please tell me what you did for the consent?
I have VISA appointment day after tomorrow and I am still not complete sure about this.
