Family visit visa

Hello,
Can someone tell me under which tier does the Family Visit Visa fall? I'm trying to read about all conditions and requirements but can't find much,,, I've applied 4 times and been rejected, I'm trying to find out where I'm going wrong.

What were all the reasons for refusal??
