i offered visa as a medical doctor from 1-8-2018 to 1-8-2019 for my COS but in the conntract my employer later offered me only 6 months employment which I started in 1-10-2018 and I entered the uk on 12-9-2108 so on 1-4-2019 my contract will end but my visa will still valid till 1-8-2019 my questions are
1-i know that the total tier 2 is only 5 years and few days so if i found a job and i started from april 2019 is the old visa will counts as 1 year of the 5 years or just the period from 1-10-2018 to 1-4-2019 which is my employment period?
2-for my new job COS IS IT BETTER TO ASK HIM TO MAKE MY START DATE FROM 1-4-2019?
3-IN MY CASE for the new job how many years and how many times the employer can give me tier 2 visa?

A few things:
I don't even think it was allowed to start from 1 Oct 2018, when your CoS states you should start from 1 August 2018;
Your visa will not be valid until 1 August 2019, your employer should inform the Home Office that your contract has ended, leading to the Home Office curtailing your current visa;
Your 5 years will count from 1 August 2018 (in theory) to 1 August 2023, regardless of how many Tier 2 you have had in between; As your employer will inform the Home Office that you have left, your actual visa in theory will become 1 August 2018 - 31 March 2019/14th April 2019, something like that;
Your new employer should make your start date 1 April 2019;
In your case you have not listed all your things such as your salary, SOC etc, it can be up to 3 years, or up to 5 years, it also depends on how long your contract is!!!! If your contract ends on 31st Dec 2019. Then it will given as 14th Jan 2020 (or something like this);
Please note your new employer should complete Resident Labour Market Test, if your job is not on the Shortage Occupancy List.
