Tier 2 general application form

Hi ,

There is a question which says Have you applied for leave to remain in the UK in
the past 10 years? in the tier 2 application form

does tier 5 extension apply as leave to remain or does it mean only ILR becuase I applied for tier 5 extension earlier
should I answer yes or no?
Please respond as I need to complete the form

In the documents required it says that I require a criminal record from uk also because I worked there earlier

I have a previous dbs check from 2016 when I was employed there

Should that be enough or anything else is required
