You would have to chose the most recent date when you were present in the UK five years before. For example, if you re-declare on 30th January 2019, you must have been in UK on 31st January 2014, and you must ensure your documents support this, i.e. passport stamps. Don't forget, their is only a two-month window of opportunity here.

28th January 2014 - 27th January 2019. Is this fine? Is it in two month window opportunity?

I have no idea where you were on 28 January 2014.

Just send the letter to UKVI as suggested.

On 28th Jan 2014 I was here in uk. I went abroad on 30th January 2014.
