Tier 2 dependant now on ILR dependant

Hi,
I have recently got an ILR visa after i completed 5 years on Tier 2 visa. My wife is on Tier 2 dependant visa since march 2015. Her visa is due to expire in July 2019. Can someone please answer the following questions:
1. What category should my wife apply for her extension?
2. Assuming that she will apply for a family visa extension, how long will she get her extension for (would a max of 6 year rule on tier 2 still apply?)
3. How much health surcharge will she have to pay
4. Will she be required to provide 'knowledge of english' test certificate even though she has been in the country for over 4 years?
5. Is she required to maintain bank balance for maintenance of funds? If so, then how much?

PBS Dependent extension.
2. Assuming that she will apply for a family visa extension, how long will she get her extension for (would a max of 6 year rule on tier 2 still apply?)
She will get a 3 year extension. The 6 year cap on tier 2 ONLY applies to the Tier 2 main visa holder and not to dependents.
3. How much health surcharge will she have to pay
£1200
4. Will she be required to provide 'knowledge of english' test certificate even though she has been in the country for over 4 years?
No, not for an extension.
5. Is she required to maintain bank balance for maintenance of funds? If so, then how much?
£630 for 90 days.

Many thanks for your help. Can you please further clarify:
1. When my wife completes her 5 years (in march 2020) and applies for an ILR, will she get a refund of health surcharge of 2 years that she has paid in excess?
2. Where can I find more info about the knowledge of English test and there is nowhere on the website it mentions that a dependant is not required to or exempted from providing this. Bear in mind that when she applied for her tier 2 dependant visa in 2016, there was no such requirement hence she has never submitted any requirement for knowledge of English.
3. Re the bank balance, am I required to provide a 3 month balance of £945 as well in my bank account together with my wife's £630 or just £630 in her account as its her application.

Unlikely.

You will not get a refund if:

your visa application is successful but you do not come to the UK
you leave the UK before your visa ends, for example to make a new application
you’re told to leave the UK before your visa expires
you’re applying for indefinite leave to remain or changing to another visa where you do not need to pay the IHS

She does NOT need to meet the English language requirement for a PBS Dependent extension, I have already stated this. PBS Dependents have never been required to meet the English requirement until they apply for ILR.

My response was quite clear was it not?? £630 for 90 days. Only for dependent.
