Postal route closed late last year.
No IHS surcharge for ILR.
Please read ILR continuous residence guidance and the guidance specific to your current visa for more info. A number of questions such as the ones stated have been answered a number of times on the forum. A quick google search will help much.

It is unclear what visa you hold exactly.

What is your full immigration history and visas you have applied for?

That a bit depends whether you apply by SET(O) online form, or SET(M) online form. These are different. You have not described your immigration history clearly so it is difficult for moderators to advise which online form you have to fill and what supporting documents you exactly need to provide.

Scanned copy of her passport.

Only a copy of her passport not the original? Nothing else to include?

You really need to up ur reading skills. Start a dummy application online and revert back if you get stuck.
The process has changed. All documents are scanned and uploaded to a partner site called UKVCAS. That means physical documents are no longer posted to HO.
