Courier Pickup Fax Letter format

Hi,
I got a call from my CW today to arrange for a pickup. She asked me to send a fax authenticating the pickup agent and time they would be picking it up.

Do any one have the format of this fax letter... I need it urgently...Please some one help me out with this.

I just sent the Fax letter and CW has achwd it... so i thought it might be useful for some one if i post the letter here...
