The extension can be applied for within the period of 28 days from completion of 30months/2.5yrs stay in the UK till the date of expiry stated on the BRP.
You calculate your 30months/2.5yrs in the UK from the date of entry in UK with your spousal vignette.
the vignette in the passport or date of entry stamp in passport?

the vignette in the passport or date of entry stamp in passport?
I can see a valid from to valid till part in passport (had a 1 month expiry)

Not correct.
It is one of the possible dates, up until the visa expiry date. The other one you've mentioned is the recommended earliest date (to avoid a shortfall for ILR).

The extension itself can be applied for at anytime.

BRP has expiry of March 2021 because though it is stated as a 30 months/2.5 years visa, it actually last for 33 months. This gives ample room for the abroad spouse to arrange departure and enter the UK with no likelihood of shortage of time when it comes to extension/ILR.

For securing ILR you will need to complete 60 months minus 28 days from the date of arriving to UK. First spouse visa is usually is be granted for 33 months which can be renewed anytime after spending 30 months minus 28 days until the date of expiry. In case of success, you will be granted another 30 months which will complete the stipulated qualifying period of 60 months for ILR. Hope it clear you now if you read and understand it in its entirety.

July 8th 2018 was date entered into Uk and is the stamp in passport. So do I apply in December?

Apply no sooner than within 28 days before 8th January 2021 and the latest, before visa expiry date.

The english b1 test is it simply pass or fail or are there scores
E.g. 4.0, 5.0 etc

IELTS for UKVI has several types of test varies from one category of visa to another. However, for spouse visa application any of these can be used provided the sufficient level has been achieved. If you book B1 IELTS life skills then it won't have any score rather a pass/fail result.

Thank you that makes sense now. So b1 test is just listening and speaking by the looks of it. Me or my wife have not found many example papers etc

On a separate note is there a checklist of what needs to be submitted for spouse visa extension application.

I can think of english language skills and financial but it would be good to have full requirement list
