You cannot edit the application once submitted. Add a brief covering letter with your supporting documents.

Hopefully it should ask you will be addressing the fact that there was an error. And in the cover letter state the correct reference number and write case worker to check the scan of the full certificate which you have uploaded.

Yes, the third party consent forms, giving the Home Office authority to verify information with third parties.

But i heard the only form that is required for set M is spouse declaration form nothing mentioned about consent home office form, is this true ?

Yes, there are consent forms to sign. I can't remember exactly where they arise, it may be at the end of the application or with the various checklists the system produces at that time. So you should come across them.

I went through the application many many times and I can't see any link to download these form and as I mentioned before the only form that is available to download is the spouse declaration unless it's not required for Set M applications ??

Those verification consent forms have been transformed to a tick box thing at the end before printing documents checklists. Like those “I Agree the TermS& Conditions“ thing for online shopping. I have submitted mine Set(M) yesterday too. Only the spouse consent form I can download and print.

Yes, printed, then signed, then scanned and uploaded.
