Tier 2 Dependent child extension query

Hi,
Please help with the below query.

My spouse has already got ILR and now Iam applying for extension of Tier2 dependent partner along with my kid for tier2 dependent child.

Please suggest me with the below questions

A) Medical Treatment (We have visited to GP several times)
1) Have you ever been given medical treatment in the UK?
2) Were you told that you had to pay the hospital, clinic or doctor's surgery for your medical treatment?
3) Where did you go for your previous medical treatment in the UK?
4) Name of hospital, clinic or doctors's surgery Address
5) When did you start receiving this medical treatment?
6) When did you stop receiving this medical treatment?
These are new questions just added on the updated online form on 1st Dec. You can answer as you deem fit

B) Parents Two Details (we are the first and only parents to my kid)
1) About your second parent
2) If I Select " NO" then it is asking Why do you not have your second parent's details?
Parent 1 is the mother, Parent 2 is the father or vice versa

C) Parental responsibility (should I choose my spouse or someone else and give my details)
1) Confirm who has parental responsibility for you.
I do not have the sigh to the form but sometimes it lets you choose the parent name by default and also tick “someone else” and input parent 2 details also

Parent 1 - ILR applicant on whom the application is based. Parent2 - other parent

If you have filled Parent1 & 2 both correctly, then you would get these 2 fields populated/visible in the form. Select both parents.
