Spouse visa extension Maternity

Hi there!

My spouse visa will be expiring 21/5, and i will be applying soon. I work with the NHS and I have been on maternity leave for the past year and due to return to work in the first week of June.
My question - can I use my 6 months payslip and bank statements prior to maternity leave as financial evidence?
also I should be paid a months salary by the time of application(accrued annual leave) do i also need to attach this payslip as evidence that i will be returning back to work?
I received maternity pay for 6 months during my maternity leave. Do I need to declare this as well?

Also I have only two documents in joints names as evidence of cohabitation. I understand we need three in joint names. How many do we have to give in single names to make up the required documents please.

I received maternity pay for 6 months during my maternity leave. Do I need to declare this as well? It will only buttress the fact that you were on leave and getting maternity pay.
