Regarding tier 2 general extension

I got ILR 1 week back. My wife is on tier 2 general dependent visa and she have not completed 5 years yet. I’m planning to apply for her visa extension,but I’m got confused for which route i need to go. I have two queries,
1) Can some one please share the link for tier2 general dependent extension form?
2) Which m BRP details I need give in her form as o have both tier 2 and ILR BRP?

Hello All,
I got stuck on tier 2 general dependent extension form and need help.
I'm applying tier 2 dependent extension for my wife. She got stuck in India for 8 months in 2020 because of Covid situation and everything was lock down. I have few questions regarding this.

1) Do I need to mention India address for option to fill last 2 years address?
2) My wife did baby delivery in UK, so do I need to give details of this in the section of Medical treatment?

Can some one please help me on this ASAP.. as my date of submit the application is very close?
