Dependent Visa and Doctor

I am on spouse visa in the UK since 2017 and currently working in NHS as a doctor in non-training post as my BRP says 'work permitted'.
I want to know if I can start training as a doctor on same visa without affecting my ILR clock and without switching to a different visa?
