Could anybody share the parent consent letter format for surrender of child Indian passport.

Sub: Letter of consent for submission of daughter's (XXXX) surrender of Indian Passport application by my husband (your name).

Respected Sir/Madam,

This is to bring to your kind notice that I, (Wife Name), along with my husband (your name) am making an application for surrender of our daughters Indian Passport as she has been naturalised/registered as a British Citizen. on [date] and obtained a British Passport on [date]

However, due to unavoidable reasons I would be represented by my husband i.e. (your name), who would be carrying our original marriage certificate & other relevant documents with self-attested photocopies of the same.

Below mentioned are the relevant details for our daughter:

Nationality:
Date of Birth:
Place of Birth:
Indian Passport No:
Place of Issue:
Date of Issue:
Date of Expiry:

Kindly accept this letter as an authorisation for my husband to represent myself and I hereby attest his signatures.

Yes you may provide the same.

My daughter holds British passport now. So, I assume need to provide to British passport details for 'date of issue' and 'date of expiry'. Please can you suggest.

The letter is for representing your wifes (consent on her behalf) while surrendering indian passport. If you want to mention date of your British passport you may do so but its not required here.

All those details would be filled in the application form.
