Copy of "Declaration of fidelity to the nation" form CTZDF

Hi All,
I got the citizenship approval letter along with the Declaration of fidelity to the nation form that I need to fill it and send back to the department of justice.
I put the wrong date in the form and want to fill it again. Tried to search on the internet and it is not available.
Does anyone have a copy of this form?

it should be filled by whoever witnessed your form.
